No Surprises Act

Good Faith Estimate

Know what your care is expected to cost — before it begins.

KNOW YOUR RIGHTS

Your Right to a Good Faith Estimate

Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining how much your care will cost if you are uninsured or choose not to use insurance. You can ask us for one at any time.

What Your Estimate Includes

Your Good Faith Estimate lists the services we expect to provide and what each is expected to cost — the sessions themselves and any related services you and your clinician plan together — along with our practice information and the dates the estimate covers.

An estimate is our best good-faith prediction based on what we know when it is written. If your needs or your plan change, you can ask for an updated estimate at any time.

How to Ask for One

You can request a Good Faith Estimate whenever you like — before you schedule, while you are still deciding, or after services begin. When you schedule a service at least three business days in advance, you will receive your estimate within one business day of scheduling; for services scheduled ten or more business days ahead, within three business days. If you ask for an estimate without scheduling, we provide it within three business days. To request one, reach out through our Contact page.

If Your Bill Is Higher Than Your Estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be able to dispute the bill through the federal patient–provider dispute resolution process. For more information, visit www.cms.gov/nosurprises or call 1-800-985-3059.

Keep a Copy

Keep a copy of your Good Faith Estimate in a safe place — it is what you would refer to if you ever have a question about a bill. We keep a copy on file as well.
